Dr Ashwin has accomplished all his medical education from most prestigious institutes of India. He has extensive training and experience as consultant at Tata memorial centre for 5 years. He has done HBNI Fellowship (only accredited course of Orthopaedic oncology in his time) from Tata Memorial hospital Mumbai. After that he established and headed Orthopaedic Oncology unit at Tata Memorial Centre Varanasi. He has fine-tuned his advanced surgical skills from Orthopaedic oncology unit at University of Essen Germany. He has done his under and post-graduation from B J Medical college and NHL Municipal Medical Collage Ahmedabad.

Scope of Treatments

Osteosarcoma, Ewing's sarcoma, and chondrosarcoma are the main three types of bone sarcoma. These are cancers of the bone, in simple language, which can spread to other
parts of the body if left untreated. However, over time, the 10-year overall survival rate of these patients has increased to 70%, which means up to 70% of the patients can be cured if the disease has not spread anywhere else in the body at the time of diagnosis. We are able to save more than 85% of the limbs affected by sarcoma with various methods, such as reconstruction with mega prostheses, cryotherapy, ECRT, pseudoarthroplasty, cement spacers, or many other techniques. Most of our patients reach pre-disease activity levels. Chemotherapy is an essential part of the treatment for osteosarcoma and Ewing's sarcoma, and sometimes radiotherapy is also required as part of the treatment for Ewing's sarcoma.

Minimally invasive treatment of benign bone tumors: Osteoid osteomas and small chondroblastomas are amenable to minimally invasive surgeries, such as CT-guided drilling,
radiofrequency ablation (RFA), or microwave ablation. The advantages of minimally invasive procedures include shorter hospital stays, immediate ambulation, and very short recovery periods, as they damage the least amount of bone possible.

Giant cell tumor of the bone (GCT), osteoid osteoma, chondroblastoma, enchondroma, chordoma, myxoid fibroma, and osteofibrous dysplasia are common benign bone tumors,
but the entire list is quite long. These are the tumors that can progress or recur locally and can weaken the bone; however, they have a rare possibility of spreading elsewhere in the body, hence they are called benign bone tumors. Osteoid osteoma and some of the chondroblastomas can be treated with minimally invasive procedures, while others may require conventional open surgeries. A few of them can be left alone and observed over time. Minimally invasive procedures include CT-guided drilling, radiofrequency ablation (RFA), or microwave ablation. Open surgeries may include extended curettage of the bone (saving the bone), resection of the bone (sacrificing the bone), and reconstruction using various methods like bone grafting, cementing, plating, or mega prosthesis. Chemotherapy or radiotherapy is usually not required for the treatment of benign bone tumors.

Soft tissue sarcoma is a cancer that arises from muscles or other connective tissues, which can spread to other parts of the body if left untreated. Common soft tissue sarcomas
include synovial sarcoma, spindle cell sarcoma, undifferentiated pleomorphic sarcoma, liposarcoma, myxoid liposarcoma, and epithelioid sarcoma, among others. Each of these has a specific method of diagnosis and treatment; therefore, diagnosis based on specific histology is crucial. Wide excision indicates that surgery is the mainstay of treatment. Radiotherapy is often required after surgery, and the necessity of chemotherapy is determined based on the patient and tumor characteristics.

Giant cell tumor of tendon sheath, pigmented villonodular synovitis (PVNS), fibrolipomatosis, fibromatosis, etc., are common benign soft tissue tumors. These tumors can
progress or recur locally but do not metastasize to other parts of the body. Excision, meaning surgery, is the mainstay of treatment except for fibromatosis, which is usually treated conservatively with medication or active observation. Chemotherapy or radiotherapy is usually not required for these tumors.

In cases of known cancer of any organ or blood cancer that spreads to the bones, it is called bone metastasis. In such cases, the bone may become fractured or may cause pain
due to weakened bone, even if it is not fractured. This requires different surgical procedures than conventional traumatic fractures, such as curettage plus nailing, plating plus cementing, or resection of bone and reconstruction with a prosthesis or prophylactic fixation. Apart from surgery, bone metastasis requires specific treatment strategies depending on the primary cancer, including radiotherapy, radioactive iodine, or Denosumab, which require tumor board discussions involving multiple experts and discussions among them, i.e., orthopedic oncologists, medical oncologists, radiation oncologists, and surgical oncologists.

Patients who have undergone multiple surgeries for non-oncological purposes, such as infection or non-union, may require limb amputation to eliminate the underlying
condition. In such cases, we can offer limb salvage surgery (to preserve the patient's hand or leg) and reconstruction using a cement spacer, megaprosthesis, or rotationplasty.
